I've had the same friends for years, and I have no clue how to introduce myself to a certain qt friend of a friend. We have a class together, and even though we sit 5 feet from each other, I haven't said a word to her. How the hell do I start talking to someone?

The "Hi my name is" is a great way to start of. IIf you don't feel confortable about it, you can find something you guys have in common, or do in common and bring up to open the conversation. You guys sit closeby, so you do have many things in common... Tests you make, difficult parts of the subject to study...
You can also use the situation in your favor. She comments something, you follow up.
I'm having a hard time thinking of examples... But the point is: the opening line won't even be remembered, unless it's epic. She won't think "I don't want that guy, he openned our talk with a shitty line", that just doesn't happen.

People are always defensive when you first talk to them, give them a phrase they can easily answer and once you've broken the ice it's easier to talk in the future.
