I still can't quite understand what you did OP, you're not explaining it very well. Did you try to log into your insurance account with the wrong email address too many times and get locked out?

If it's free why do you need your bank card? How does your bank know the name associated with that email address? Just from the name you submitted to the dating site?
It's expected people have more than one email address nowadays, so I'm guessing that the real problem has nothing to do with your email. It's more likely that you creating a dating site account with your card and a different name put up a red flag for them. If a site asks for your bank info up front they probably intend to do some test transactions to see if it's a valid account. If these came in under your fake name, it would look like somebody stole your card.
