I forgot to note that she has expressed that she wanted to relocate to somewhere tranquil to live with her boyfriend away from society. Sort of like a Buddhist monk even though she isn't a Buddhist. Is this also something I should worry about? She also invited me to come but I am not sure if I could live that lifestyle if she was serious or not.

My circumstance was entirely different, but I dispalyed the same behavior as your sister once I left an abusive environment. I was 21 by then, but I had never been able to experience chidhoold and teenagehood properly.
I then had a phase in which I played with dollhouses, and even experienced a bizarre psychological sort of second puberty a few months later.
I noticed my own strange behavior and looked for help. My psychologist and psychiatrist both affirmed that it's normal taking the time to try to experience that which we couldn't safely do, but still perceived as essential, once we felt safe.
I think the situation is the same for your sister. She felt deprived of the childhood she wanted by being trapped by stereotypes, so now that she can, she is making up for it. It will pass when she's contented. My make up childhood only took about a year.

She was involved in an abusive relationship with her ex-husband and I think this has reflected on her for a lot of things. A reminiscence about her childhood and longing to return. For the sucidal tendencies and the inabiity to function properly, you may have cause to have her committed at some point if she really becomes a danger to herself. You should have a doctor you can call during an episode. But even then, you may go to therapy yourself and talk about your mom. No but I do know John Hopkins hospital which was one of the first hospitals in the US to do that surgery now refuse to because it has a crazy high mortality rate. Well I'm pretty hesitant on people getting gender reassignment surgery because I get the feeling people don't understand the gravity of what they're doing till after, but in your sister's case she sounds pretty stable and content. If you want you can try to monitor her behaviour and make sure she's okay, but you have to remember it's her life to live and her decisions to make. Surgery has already been done, there is no turning back.

But if she does generally seem socially adjusted and from the way you put things everything is going well with her maybe you can chalk up playing with dolls as an eccentric hobby.
