Looks infected, possibly from biting your nails? If you're an americuck I suggest getting a lighter and a sewing needle and lancing it. Use some rubbing alcohol or peroxide to sterilize.

Yeah, that's an infection. I get those when I bite my nails or tear a bit of the side flesh off. Get some rubbing alcohol or anything that extracts fluids. You need to get the infection out of there, usually your body will do this for you if you keep it clean (which is troublesome because we use our hands a lot). But try to keep your hands as clean as possible so your body can force the infection out and heal.
