Depends on what he likes.
Make a list of all the things you know about him: what are his hobbies, what he likes, what he does in his free time, inside jokes you have, whatever.
Find something related.

I always go for a book about a subject he's interested in, a small gift with a meaning for us (something I made, a reference to an inside joke) and something useful. Usually works.
