You need discipline. Start marking things on your calendar for every day of the week and actually do them. Keep it up and theres your discipline.

Initiative and discipline are important. If you have a job, try to transfer some of that to your day to day life. Do whatever it is like you're getting paid, and you will eventually be paid with peace of mind. I really like the calendar idea.

But discipline is a skill. You don't just suddenly work for hours everyday. It's like a muscle, first you do a little bit, to be able to say "I do X every day", also time allocation is key; 30 minutes after dinner, before going to sleep. Cut off all distractions.
One key point of discipline is doing it even if you don't feel like it. Just motherfucking do it.
Anyway, you start by doing it for a brief period every day, and then you slowly raise the bar as time passes and you get used to doing it.
If done right, your life pretty much flips around in a relatively short period.
Also try to cut off on one of your main vices, for example porn (not necessarily jerking off unless it's not much of a problem for you). In my case I'm trying to reduce my coffee intake because it was a liability for me.

Just depends on what you are trying to do. If you're going to run do it for time, chores just do them, read x amount of pages yada yada.
