So I swallowed a metal screw around four years old, nothing ever happened, would it still be in my stomach, and if so, is there anything to worry about?

There is a 100% chance that the screw is still in your stomach. Everyone knows that when you put things in your stomach it stays there.

It is a medical fact that the body has no natural way of gathering whats in your stomach, processing it, and then pushing the contents out through an exit of some kind.
