Can anyone help me read this prescription? I'm trying to order some lenses at zenni but I don't know if it's missing information or what.

They ask for the Sphere, Cylinder and Axis of each eye. Thanks in advance.

Call your optometrist and request a printed readout of your exact lens prescription.
If they won't give it to you, get a new optometrist.

Or you could order incorrect lenses, do what >>17809216 says then send them back and complain that you fucked up and maybe they'll send you another pair for the same price.
