My group in college thought it would be an interesting idea to incorporate a fun type of survey or questionnaire for fetishes and kinks for our fetish presentation. Just to kind of give an idea to the students in our sex ed class about who they are sexually, what they would like, or if they have a problem. Any advice on what I can do to come up with such a survey or where to find a decent pre-existing questionnaire that I can print out and hand out to the students in class would be greatly appreciated.

But idk if you've ever seen the kink charts on /soc/, you might be able to draw inspiration from that. Basically just asks people to check off their kinks with colors, green for "love it" yellow for "Like it/willing to try" and red for "Absolutely off the table"
Personally, I think it'd be interesting to break it down and see where people's boundaries lie in that sense. Find out what people are willing to try even, even if they don't particularly know or think they would /enjoy/ it, but just not hate it or something. You could phrase it like "Suppose your loving partner expressed a desire in acting out X fetish, would you be willing to try it for their sake, or are you completely uncomfortable with it"
