What's the best way to deal with vaginismus?

Women with vaginismus can do exercises, in the privacy of their own home, to learn to control and relax the muscles around the vagina.

The approach is called progressive desensitization, and the idea is to get comfortable with insertion.


First, do Kegel exercises by squeezing the same muscles you use to stop the flow of urine when urinating:

Squeeze the muscles.
Hold for 2 to 10 seconds.
Relax the muscles.
Do about 20 Kegels at a time. You can do them as many times a day as you want to.

After a few days, insert one finger, up to about the first knuckle joint, inside the vagina while doing the exercises. It's a good idea to clip your fingernails first and use a lubricating jelly. Or do the exercises in a bathtub, where water is a natural lubricant.

Start with one finger and work your way up to three. You'll feel the vagina's muscles contracting around your finger, and you can always take your finger out if you're not comfortable.

For women whose vaginismus is related to fear or anxiety, therapy helps.

I also have vaginismus. I also have vulvadynia. So, it is possible only part of this applies to you.

Vaginismus is sometime developed by a trauma in your life. Was there a childhood trauma? Alternatively have you been raped?
The tenth time in my life (approximately), I was involved in a non-consentual sex scenario by my then-boyfriend. He was my second partner. I don't remember having sex issues with him until after the non-consent issue. However, I had previously had issues with the past partner because he couldn't get inside me and he would force himself inside me, and I let him.
Anyway, trauma can do it. But it isn't necessarily the reason. If it is though, you should do talk-therapy. Ideally someone familiar with sex therapy.

When I first went to the gynecologist she did several tests to determine the issue. She saw micro-tears at the entrance of my vagina, showing that It was visible how partners had been forcing themselves inside me. Since that was all I knew, I didn't even realize it wasn't supposed to be painful like that.
What she also was able to see is how I flinched with pain when she poked me with a long q-tip. She did the same motion on my arm, and she was not pressing hard at all. The third visit I nearly had a panic attack when she brought out the jar of q-tips (kinda like the panic one might feel if it was a needle). There were certain locations checked and this is how she diagnosed me with vulvadynia. The very entrance of the vagina she was counting as part of the vulva I believe.

The other exam the doctor did was for vaginismus. For this, she inserted her fingers inside me and pressed on certain muscles. She pressed in many directions, all of which were painful. There was one area where I was in so much pain I thought I was going to pass out. She told me I had weak pelvic floor muscles. She said that every time she pressed there was an involuntary muscle spasm, that may have been causing me pain.

The doctor I saw was an OBGYN who was a "Pelvic Floor Specialist" another search word would be "Gynecological Pain".

Anyway...the diagnosis I was given was vulvadynia, vaginismus, and dyspareunia (pain with intercourse).

The doctor's preferred treatment option was Physical Therapy. This is called Pelvic Floor physical therapy. These specialists are fairly common.
Unfortunately my obnoxious insurance didn't cover any condition related to dyspareunia in females (super sexist, since they did cover dyspareunia in males), so I was not able to get physical therapy. I did get one dialator from the physical therapist during the evaluation appointment, but I really found the exercise unhelpful. It was just inserting this dialator no thicker than a tampon inside me. It didn't really hurt inside, it hurt at the entrance.

Option 2 was medicine. The doctor perscribed two compounded medications.
1. A vaginal suppository containing 5mg valium and 6mg baclofen
2. A cream (1mg per use) to be placed in the vaginal area containing 2% baclofen, 2% elavil, and 5% lidocaine in a petrolatum base.
My sexist insurance rejected these too. Althouth their bull shit excuse was that they couldn't fill it because the pharmacy benefits didn't pay for over-the-counter drugs (the bases in the two compound medications...petrolatum is sold as vasseline)

Instead, my doctor had to perscribe me valium, the kind you would take orally. She said I should insert it vaginally and then remove it after 8 hours. It was super embarrassing to pick it up at the pharmacy because the technician thought the directions were wierd, they didn't understand why it would be inserted in my vagina. Anyway, they also told me not to flush the part I pull out, they recommended I bring it to a fire station. Like wtf?? You want me to bring in half-absorbed pills of valium that had been in my vagina for 8 hours?

Needless to say the valium was not a successful treatment.
I was perscribed 10mg Noratriptyline to take orally each day, which is supposed to take the edge off the chronic pain.
The next calendar year, I switched insurance and was also able to get the compounded medications.

The compound medications really work! I do recommend them. The one issue with the one with lidocaine in it is that that's a numbing agent.
What is the point of having sex if you are numb?

In terms of sex, I noticed I tighten very easily and it is hard to relax me. If I have sex every other day, my body can adjust to that guy. But...it can't be less and it can't be more or it hurts a lot.

I hate to tell you this anon, but valium is already in the water supply. So are a lot of other things. Hell, the main way of disposing cocaine is flushing it. The water supply is both naturally and artificially filtered. That valium wont harm anyone. You can read more about how the water supply is filtered by googling it.

I've only read what I've seen on google so I agree with the first anon. I'd say be consistent with it though until you see results. I had an ex who had it but she never took any steps to correct it, so it's good that you want to make the effort.
