I want to improve my spoken English , I can handle text conversations easily since there's no pressure or something like that, I can't find someone to talk with over internet (I live in a country where there's no one who actually can handle English.. sigh)

The reason ESL is divided into four skills is that listening, speaking, reading, and writing are four different neurological processes. You can use isolation tasks or compound tasks. If you're looking to improve a specific skill, isolate it. There's also a weird combination task that helps form the neuromuscular connections in speech, and that's reading aloud. When you read aloud, your mouth gets used to making the phonemes in the language and stringing them together in words or sentences.
Speaking itself can take on several forms. Phone, conversations, monologues, presentations, etc. Specifically which one are you having problems with?

Conversations, I struggle sometimes when I speak with someone, I just fear that m pronunciation might be wrong or my grammar is wrong, or the second person doesn't really understand me, for example when I'm in the mid of a sentence, I feel like "Did I make a mistake, Is my pronunciation right..." and similar ideas.

The only way to improve that is by having conversations. Get a hold of some university kids or Muslim converts or ISIS supporters in the US or England, do a language exchange with them. The only way to learn conversation is to have conversations. Use a book like Compelling Conversations or Taboos and Issues.
