I fucked up and entered all my information into a spam form. What do I do to stop the person who got it from doing anything bad with it?
By information I mean litterally everything important down to mother's maiden name.

You can't take it back. What you need to do is change your passwords and security questions for everything you can think of, from your bank accounts to your old Neopets account.
One, why would you enter all that stuff?
Two, change your passwords. All of them and starting with your email.

i've been doing a lot of things lately like student loans that ask for that information so i assumed this was similar. I'm also very young and inexperienced with government forms and it looked legitimate.

You're going to have to either manually keep your eye on your identity and credentials or hire a company to do it for you for at least six months. Anyways, good luck and I hope it works out.
Change all your passwords to something so hard you have to keep a list of passwords at your desk. Financially I'd talk to your bank and mention your fuck up and see what they'd want you to do.
