I've had a few.
There have only been two methods that helped
1) be patient, and maybe it will grow past the flesh of your toe. My wife offered to clip my toenails a few months ago, and she went too short. It was awful, but they grew out and the issue went away. Both big toes.
2) Get a Doctor involved.
I had a bad ingrown toenail in highschool, and it required having my toe numbed so he could clip away some toe meat, then cut the nail. I was so happy to be out of pain, I didn't even care (couldn't run, debilitating pain if it got stepped on).
In High school I tried using a tooth pic to "pry the nail above the toe and "train" it, but it never worked, and was pretty painful.

Soak your feet in hot water, to soften both skin and nail. Then try to lift the edge of the nail away from the spot where it's growing in. Do twice a day, and eventually you might "train" the nail away from the ingowing part.

Been there, done hat. They pump that shit so full of lidocaine that you really do feel absolutely nothing. It's not like dental anesthetic, where you can still feel the dentist mucking around; with the stuff podiatrists use, it's like your foot isn't even there.
Also, they put up a shield so you don't have to look.
I got it done and they put me under general and local anesthesia. I guess it depends how bad it is and how good your doctor is.
