If they kept you waiting for an hour that restaurant is Gordon Ramsay tier terrible.
She was warning you against dishes, which means there must be something going on. The mushrooms were probably mouldy, and the other dish just bad. In hindsight she should have let you eat it anyway, but she didn't know if you were a cool person or not. This is why you all feed each other to the dogs in the US.

If i heard that at a family restaurant like Carabba's when ordering a staple of the menu, i.e. any chicken dish with mushrooms in it... logically speaking, it can't mean anything other than "I don't want to serve you". Seriously, what else can it mean?

I'm not comfortable forcing people to serve me if they don't want to, so if possible, I would leave pretty quickly without ordering anything.

You were probably the waitresses' last customers at the end of her shift. The creme brulee takes awhile to make if it's not pre-made, and she probably wanted to clock out. Who knows? It's not like the waitress was rude, just a dumb busybody. Don't think it merits a complaint.
