Well I've got two main ideas, the first one being a point and click 'J-horror style' game, so pixel art and solve the puzzles to advance the plot.
Basically the main character is a western dude with a Japanese Grandpa who remained in the US after the war, he recently died and the protagonist is visiting the island he came from off the coast of Japan to scatter his ashes and find out about a local legend that all the women on the island disappeared over the course of the war.

Other one is a Hillbilly Horror simulator/Mutant Red Neck Clan management game, run your clan, capture women folk for breedin' and breaking in, sire inbred offspring with your sisters, maybe a bit of a research tree type deal where you can choose to go full cult, mutant or take over a small town to act as a trap for people (Or keep your bloodline normal lookin' enough to send women/men off to nearby cities to lure in potential stock)
And of course trying to avoid the feds, people that come looking for their wives/daughters/girlfriends and so on.
Basically totally bastard monster rape simulator with you as the monsters. Each turn being a month or maybe 3 months or something.
