Mind Flayers start life as a tadpole swimming in brine. They eat other tadpoles until they're big enough to take part in a ritual where they eat a medium sized non-dwarf creatures brain and subsume it's body. The from the far future is pure conjecture in universe.

Part-Mind Flayers are either incomplete transformations or aberrant experiments.

Well yeah. But the main body structure is till there. That's where all the half-whatever flayers come from: putting a mind flayer into something that isn't a human or an elf. Also, that far future stuff is spelled out in a 3.5 splatbook iirc.
