It depends entirely on the scope of your game.

If you start out with just a mechanic in mind, like "quadruple amputee erotic wrestling," then you don't need to design the entire world first. In that case, you just work on the mechanics, attach them to characters, put those characters in a world, etc. And hell, for a game like that, you wouldn't even need a world. See Night Games Mod for an example of a game that was designed around a mechanic.

If however, you're planning to design something like an open world rpg, then yes, I would suggest ironing everything out before you start developing. Not just because you want a clear picture of the game, but also to prevent scope creep. Chart out everything that goes into v1.0 first. When you're done with that, start development. If you decide that you want to add X, Y, and Z while you're working on it, add that to a list of features for v1.1. Too many games get side tracked by crap the devs decided to toss in halfway through development and have to change things around to accommodate their existence.
