Games by Black Isle and other spin off studios after interplay died made some fantastic open world games like Fallout 1, Fallout 2, Fallout Tactics, Arcanum, Baldurs Gate Series, Icewindale Series...

Open world games these days in first person RPG's just mean you can just fuck around playing mine craft in the game world.

Rather than forcing a family on you, the player could be a mercenary contracted to break into a vault and steal the vault dwellers kid. You could choose to abort the mission and rescue the kid and become enemies of the institute and set one story path with branching story lines or give him over to them and become their allies which starts another.

As far as I know, there's only a brothel in one of the towns where you can hire one of six prostitutes (3 male, 3 female) for a fade to black situation. There aren't any rape scenes or anything like that.
