I think the life time cost doesn't apply for previous usings of Emperor Time.

Scarlet eyes =/= Emperor Time. He can freely turn them red without any repercussions. I don't know if it boosts his fighting skills that way, without emotions, but he doesn't achieve 100% proficiency without Emperor Time. He wouldn't know the exact time if he wasn't the one that put the restriciton.

Consider the following abilities of Kurapika's that require his eyes to go scarlet (to eneter Emperor Time) to use:
Judgment Chain (律する小指の鎖 The Arbiter Little Finger Chain): Kurapika must enter into scarlet in order to use this ability. He sets two conditions onto the target and if they voilate at least one of them, they die.
Dowsing Chain (導く薬指の鎖 The Guiding Ring Finger Chain): Whenever he enters scarlet eyes he can detect when someone is lying, other than that he can use his other abilities using this chain freely without entering Emperor Time.
Stealth Dolphin (人差し指の絶対時間 The Index Finger's Emperor Time): After using his index finger's, Steal Chain, he can use stealth dolphin to analyze and equip the ability. He can then give anyone this stolen ability but they can only use it once before the ability is used up.
Holy Chain (癒す親指の鎖 The Healing Thumb Chain): Whenever he enters scarlet he can heal advanced wounds.

With all of these abilities and the amount of times he used them in the past, this guy must've lost about 20 years already (possibly more behind the scenes.)

Yeah that fight should've taken about 5 minutes and I believe his eyes were scarlet for that entire duration so he at the very least lost 300 hours in that fight alone.

That's what I'm thinking as well.
Sadly his entire kit relies on Emperor's time.
The only abilities that don't are:
Steal Chain: Which is good for disabling and not much else without going into Stealth Dolphin.
Holy Chain: Which can only heal light wounds without going into Emperor's Time.
Dousing Chain: He can just use it as a normal weapon.
Chain Jail: Great ability BUT can only be used on people he believe are phantom troupe.

The fuck are you talking about? in this arc we only saw him lose 5, and this is probably the most he's ever used Emperor Time. In all previous cases he didn't need it for more than a few minutes.

A minute is worth 60 hours each. He often activates his Scarlet eyes carelessly costing him an hour each time.
Don't forget that abilities that he used in the past rely on scarlet eyes to be activated for example; Judgement Chain. During that whole York incident he possibly lost 5 years.
We also have to consider behind the scenes where he ventured to get the scarlet eyes, do you honestly think he didn't activate his eyes once during that entire time?
20 hours sounds right, 15 hours is bare minimum.

No, it's stated that when his eyes turn scarlet, he becomes a specialist thus activating emperor time.
So yeah every time after I would say he finished training with Izunabi that his eyes turned scarlet he was losing lifespan.

Not sure how much he's used it in total, but he's had emperor time on for about 12 hours since he's been on the boat, which is about 5 years of lifespan.
