a) anime has set episode limits. 11-13 just isn't enough for most shows.
b) they get constantly reworked in production- It's possible there were ideas that would better explain certain plot elements, but never got implemented because reasons.
c) anime writers suck. Plain and simple. There's a reason why Okada and Urobutcher are memes on the internet.
