Where Counter Guardians fit into all of this is that if the scenario is dire enough that the above strategy does not work or ends up failing, a bunch of Guardians like EMIYA will be directly dispatched and pull a Samson option that kills everyone involved.

Karna's thing was explained in the novels as being a combination of his strength, skill, and a really, really high quality spear that allowed the rank of his attacks to reach A without his strength being A.

Basically consider it that the attack would do x damage and based on Siegfrieds skills with the sword it's reduced via parrying dodging glancing blows etc, if the amount of damage left over after the above exceeds Fafnirs threshold he takes the difference in damage.

So an attack Seigfried see's coming and can deflect some of the damage might not pass through fafnir, but the same attack hitting seig head on would.
