It was so fucking rushed that the English title of the show is Yuki Yuna is a Hero, but the main character is called Yuna Yuki in the dub.

The voice cast had a Q&A on youtube, where they revealed they didn't even get Togo's song and the dubbers had to basically make the song from scratch and get Togo's VA to sing it to what they made, if I recall right. Funimation's simuldubs aren't that bad for high budget, popular shows like My Hero Academia.
But with simuldubbed shows, they actually go back and redub a lot of lines for the home video release.
(Not just to correct errors, but also because reanimated scenes need to be retimed and stuff.)

Pony Canyon's YuYuYu dub was just a one and done deal.

It was Pony Canyon USA's first attempt at a release (along with a sub-only Denkigai).
If I remember correctly, Pony Canyon "USA" is still based in Japan and staffed with mostly Japanese people, so they're not exactly experts with the foreign market.
