It is a deliberate stylish decision. It is very difficult to draw a large character and a small character in the same frame. Some levels of cheating is required to portray interactions.

Trying to stay perfectly to scale can work, but that is usually timer consuming and reserved for expensive productions. For example, the Evangelion Rebuild films was the first and only time that Evas had a fixed height.
