The bullshit is harsher than the first episode of the anime. Touma seems to be at fault for everything.

The anime implied Index's habit being left behind was a mutual accident neither of them noticed right away.

Depends on what you like/dislike. The major differences between the anime and the adaption are as follows:
Scenes missing in the anime - there are two volumes with missing scenes (2 and 4) both of which contain antagonists dropped from the anime. So even if you decide to start from the later one (vol 14) it might be worth checking those two out.
Lot of missed thoughts and planning - Probably due to time constraints, a lot of the world and character exposition is lost in the anime. Touma especially gets pretty dumped on character wise.
The rest of the stuff is relatively minor higher levels of gore and more lines of dialogue here and there.
If these are things you care about, try starting from the beginning. If not, start at volume 14.
