I only see two endings for Kiyoshi in terms of romance. The first is that he ends up alone, or we get a timeskip showing Kiyoshi settled down with some new pretty and nice girl he met when he was older off-screen. we don't see her face or anything but her design from the back and description will be enough to let us know she's neither Chiyo or Hana or any girl from the school so she was never involved in all that drama.

Chiyo has never shown any romantic anything to Kiyoshi, right? She just seems really kind and Kiyoshi misunderstands it.

They went to Sumo once, went badly, and she comforted him when he was suicidal.

But the fact she thinks everyone knows it's her birthday, and instead Kiyoshis turning it into some confession event, I don't see it going well.
