If they do have kids, then Kyousuke risks prison if anyone finds out he's the dad and Kirino's brother (and the slight risk of birth defects compounds this issue). They will need to move far away from their parents to ensure no one finds out about their relationship, and even then, someone in their new hometown could put the pieces together. If they come out and admit to being a couple like Kirino seems to want, they're going to get cut out of any inheritance, and risk jeopardizing their relationships with their parents altogether.

Their best bet of living a normal life would be to change their names legally, then move somewhere outside of Japan, just to ensure no one puts 2 and 2 together and figures it out.
