Why is manga horror so difficult to achieve?
I think horror is hard to do with just pictures, period. Film, video games, short stories, sure, but, for some reason, it just doesn't look that scary when it's drawn on paper.

Nah. Pictures are fine, it's animation that doesn't translate well unless you use some very distinctive or experimental style. Maybe culturally we're just too used to associate animation with funny shit.
None of Ito's works are scary.
Except for The Bully.
Because it's hard to build suspense. When you turn the page, even if you dont mean to, you end up quickly scanning the whole page and a lot of times it'll ruin it. There's a great video about how junji ito uses the page turn for jump scares, but the main thrust of it is that he uses the last panel on one page to set up the suspense, then forces you to sheepishly turn the page knowing that some fucked up shit is coming.

Because horror is about rhythm.
Horror movies have a particular rhythm in them. They do a quick presentation of characters, set-up the fantastic element, and then the build up slows the rhythm, and the climax accelerates it.
You cannot create sequential art with fixated rhythm. You can try, with panneling, disposition of images in the page, and text flow. But mostly, it kind of doesn't work, because the page is still available for you to look at its entirety at first glance.

Because you need new absurd horrifying concepts to scare people. Ito does this really well. You normal ghost/murderer stories don't scare people anymore.
