I mean, I can spoiler the manga plot for you. The whole "falls in love with her" plot line isnt actually the twist. It was a misunderstanding and Aki never told him to leave in the first place, it was Koiwai wearing a wig impersonating her. In fact, Masamune leaving left Aki so heartbroken that she still has an eating disorder and hates all men. So there never was a point in that revenge to begin with since she wasnt the one who hurt him.

The way I read it Aki was actually a very lonely person and was going through a very difficult time when fat Makabe showed up. He gave her some happines and was basically just what she needed. Not saying it would be love foreverever but I know from personal experience that sort of thing ingratiates you with that person for a very long time.
