I don't think too much detail is anything bad. Berserk usually does that to encapsulate the sheer massiveness or scope of its setting which I think it does greatly.
Berserk always uses this to show the imposing atmosphere.
This for uh.. I don't know why, but it's one of my favorite in the whole manga. Something about guts calmly overcoming a situation with unrefined brute strength was a good mix.
This was one of the covers for the "Justice" arc. Technically, Hol Horse helped the Crusaders by telling Jotaro about Justice and how it works.
A LOT of people speculated that Hol Horse was supposed to join the Crusaders at some point because of this cover, and that Araki being Araki dropped the idea last minute.
I should probably consolidate this one of these days.

Depends on the artist. Oyasumi punpun uses a lot of photos turned black and white. Murata and Miura hand-draw basically everything. Lots of others trace photos for backgrounds.
There's a video series on youtube of Kaoru Mori drawing an Otoyomegatari poster to give you an idea of the process.
Most of Inio Asano's work consists of actual pictures put through a filter, Copy & Paste techniques, and other sorts of digital manipulation. It's just that he does it all so well you barely notice.
Most traditional works though are just the product of a talented mangaka and their assistants
It's just him being dumb and using Mangareader. Non-shit sites have better versions of it.
