Normally the rendering engine puts less effort into rendering stuff you're not focused on, but puts more effort into things you're staring at in great effort. So if you're in a fight, getting ready to commence a surprise attack, or are spying on a particular person, the server will reference that person to get more information to render. But the referencing causes lag that some players are able to feel.
It's a really weird system, if players can experience noticeable lag like this, but it's not entirely impossible.
