People tend to forget the Nanaya style is suited for assassination, not for 1-on-1 combat. Just read Shiki vs Akiha in Kohaku's route. The only reason Shiki could beat her was by using the place to his advantage, not by taking her head on (since she would have killed him).
Shiki could kill Nrvnqsr because the latter had no idea he possessed the MEoDP, and by the time he realised what was happening it was too late.

First of all FGO is irrelevant for Tsukihime as it doesn't happen in the same reality.

Second, FGO events are not canon to the storyline of the game, unless you also want to count pic related as canon and we're supposed to think that in Nasuverse servants canonically have 1-2-3-4-5 stars and if they lose a star that star turns into chibi versions that fight against servants. So no.
