Actually 228 is based on a special manga chapter (BLEACH on th BEACH). Most filler list include it which is kinda understandable as the chapter was ~10 pages long (most of the episode is original content) and has no place in continuity.
However many also label 227 (which is a composition of 3 special chapters, all in continuity) as filler, so you should be wary if you want to skip fillers.

Not exactly. What I meant is that it's good idea to watch until you see the decrease. So if you didn't like it since the beggining, it isn't necessarily good idea to drop it, since it becomes very different series once the overarching plot kicks in (episode 16). People who mostly liked the SoL part are usually disappointed with everything that comes after, while those who enjoyed action and plot of the second arc find this point much later.
So if you're fun of standard shounen long rescue arcs and big battles, you might want to continue at least until episode ~20. Otherwise, don't bother.

But I would advise you stop watching it once you finish the Arrancar arc. The series doesn't really get any better from that point on.
