in this episode he is struggling with his sexual desire for asuka, which is why her breasts are drawn larger than normal (as that's how he sees them)

you can see how anno has used this in later episodes (e.g. how much darker scenes became as shinji sank into depression, culminating in the geniusly abstract finale - even the eye colour thing is explained if you read into the text properly)
