Yukito did turn into a crow. This is something that can sort of be explained. In Kano's route, it's shown that Yukito only has a limited amount of power. If he uses it all, then he would disappear. In Misuzu's route, not only did Yukito break the curse, but he also wished to go back in time to stay by her side. There's multiple theories on what happened after this. My Theory is that Yukito lacked enough energy to go back in time completely, and thus was forced to compromise. He wanted to stay by Misuzu's side, so the best way possible was to change into something she loves. Since dinosaurs are out of the question, the next animal she is shown to love are crows.
I've read one theory in the past that suggested that Yukito isn't allowed to change what already happened in the past, and thus was forced into a role where he is powerless.

Him turning into a crow, and the ending with the children are the only things left vague.
