after he rematerializes into the real world again, he strangles Asuka. Symbolically he's trying to affirm if Asuka can accept and live with a person like Shinji despite all the flaws he has and the problems he struggles with to which Asuka strokes him in the cheek. Shinji breaks down into tears because of this.

Asuka then proceeds to say "how disgusting" but in reality she says this in regards to herself, for having accepted Shinji's faults and problems in pursuit for her own happiness, realizing that she too needs to accept other's problems and pain instead of pushing them away.
