How the fuck do people sit in these positions for extended periods of time? I've tried and can only go for like a minute before it really starts to hurt. Does it just take practice or is it because Japanese are small?

I think once you sit like that long enough, your body will adjust itself to facilitate that position.
That's why they taught it to children, because their body can easily adjusted.
If you tried it on adults, they need mind of steel before able to master it.
