I don't know if anyone else has already had this thought, but I have a theory.
I think the Future arc is another simulation. First thing that made me wonder was the red blood. Even in the Despair arc, the blood is still pink. Secondly, Makoto has been incredibly inactive as a participant - this could just be Protag Syndrome from the games bleeding into the show as a deliberate choice, but I think he might be a bit dazed and confused by a sudden change of scenery at some point that he can't figure out (being put into VR).
Why is it a simulation? The Future Foundation is testing him, putting him in a situation of ultimate despair to see if he'll crack, to see if he really did change sides or help the Ultimate Despairs.
It's also corroborated by whatsherface teacher person commenting on her own death. Though, we all know DR frequently breaks the fourth wall without any real ulterior motives.
