The game isn't happening. It's not a simulation, because it's all taking place in Makoto's head following the gas attack. It's a battle for control over his body between him and Ghost / AI / Joker virus Junko. The anime will try to make it seem ambiguous whether it's real or a hallucination, then it'll all come down to whether Makoto believes it's real (and kills himself) or believes it's a hallucination (and kills kyoko or P4 protag or something). Penultimate episode will be ambiguous. He'll make a choice and it'll cut to black.

Or it's just Monaca and that's it, no other twists.

While making Naegi suffer is probably a neat side bonus for Junko, the real goal here is to make Munakata drop the act and fall into despair. There is a definite grudge at work here, and I don't think it's really against Naegi.

I'm betting that (real) Chiaki set this up as a VR simulation, and is using a copy of Junko to run the game. Chiaki wants to expose Munakata and his Mirai friends as being less the cure to the world's despair, but instead the root of it.

Old man and the teacher probably helped to create and evolve Junko into what she became. Rest of the cast probably has their own crimes as well.

Chiaki gets to expose the Future Foundation as being just as evil as the Despair they claim to fight, Junko gets one last opportunity to fuck with Naegi. The world gets to see their 'saviors' being a bunch of kill happy psychos while Naegi gets to show the world AGAIN that he's the only one who knows what the fuck is what.

Junko's clone will convince Naegi that both despair and hope are necessary, with Munakata being a clear example of what happens when someone tries to eliminate one or the other. She herself realized this after being beaten by him- her very despair at losing had given her hope which she quashed before it could root by killing herself.

And after stopping Munakata, Junko will tell Naegi that only he, with the strongest hope, can be a match for her, with the strongest despair.

And then they will have loving hatesex.
