It really wasn't that she was shunning him, it was more that they didn't interact whatsoever for years. They lived in the same house, but other than that they mostly just kept to themselves. Then he started to get involved with her again because he found out her secret, and she figured out that her amazing onii-chan was still there, hidden behind feelings of inadequacy and uselessness, but she didn't want to admit to herself or to him that she was still in love with him because there wouldn't be a happy ending. That was the point of her fucking off to America, and the point of her trying to fuck off to Europe. In the end, no matter what she did, she couldn't fall out of love with Kyousuke, and Kyousuke interacting with her again made her more and more aware of that, especially as he regains his confidence bit by bit and does amazing things that only the legendary siscon could or would do.

At the other end of the spectrum, it only seems as if he's getting nothing in return if you're not paying attention. His "reward" is that his relationship with Kirino gets better. Furthermore, even if he weren't getting anything in return, that would be fine, since he views it as his responsibility as Kirino's sibling to take care of her when she needs help. Kirino feels exactly the same way, which is why, when he's dumped by Kuroneko and is so hurt that he actually cried in front of Kirino, Kirino takes his usual role and takes care of him. They're siblings, after all. Even if everybody else abandons one, the other will always be there. Of course, that's a bit more than what a sibling should be expected to do, but that's the point. "Because we're siblings" is the excuse. They both want to take care of the other, because in reality both love each other deeply.

First of all, it's worth noting how not all NBR is the same. If the female love interest is just a stepsister whom the MC has just met or has known for a little time, it's not even inseki imho, it's just a regular relationship between two non-related people. Things are different if the two of them have grown up together, especially if the imouto was adopted, or became part of the family, when she was a toddler or younger. Like it or not, two NBR siblings growing up together will develop the same bond as BR ones, "childhood friend" my ass"muh DNA" has nothing to do with it; however since BR incest is seen as more socially unacceptable, I can see how, at least as far as fiction goes, they belong to different leagues, so I won't say here they're the same.

My point is, there is no need to put NBR and BR in the same category. We can all agree NBR is trash when it's only revealed at the end so that the final obstacle standing between the siblings is removed, but for all the rest, it's just another kind of love story with its own features, that depend, for example, the fact the two siblings live under the same roof, on their closeness. If you argue it's the same as childhood friends' love, even if we disregard what I pointed out in the first paragraph, you're wrong, for the simple fact they're two different kinds of characters so the story plays out differently. For example: you can't say a love story about a teacher and a student is the same as one about two students just because they both have a school setting, it's retarded.
