Nobody built the walls, the first king commanded the titans to form into the walls and mind controlled everyone else into not wanting to leave and had the government purge the few that were immune.

And even then, the island strategy hinges on the fact that the titans can't swim, despite being suprisingly light and constantly searching for humans. Perhaps there is a small island colony of people who have escaped the titans, but the story isn't focused on them. Such a thing would go against the theme of the story of having to fight your way out of being backed into a corner if you could just safely stay in the corner.
