I don't know the healthcare system within the walls but Grisha was just a self-employed doctor, right? We never saw governmental hospitals or anything but remember how there were nurses/paramedics when there were corpses after Trost battles. Grisha probably worked with those people during the outbreak in their treatment centres under an order. I agree that they want to give you something from they have like a chicken or whatever they have from their own business and doctors accept it. Though if they are truly poor do you think doctors will accept when they want to give a few coins? I know doctors giving money to these people to help them, if they need to buy medicine or some food to recover. And don't think that rich doctors are the ones doing it instead they do it with an average-low income. It's up to person of course, not that we know Grisha. What I couldn't see was Grisha getting a specific price from each patient in a state of emergency if it's like his office.

I've seen in this action as well very closely, I still do. I don't saying Grisha was poor or rich, he probably had enough income but expecting him to make a fortune after an outbreak especially happened in destitute places isn't likely to me.

My assumption was over where they live since it tends to give you a clue but it is not certain. For example living in the rural areas doesn't define your economic situation automatically but Sasha and Connie is from villages of inner walls and we see Sasha's village has problems to maintain their income (or food) for example. And Annie was trained by her father with no cute exercises. Also, neither Annie or Mikasa play while fighting, so Annie or Mikasa beating Reiner during training is not awkward or forced, and threw him a couple of times doesn't mean they could win all the fights. Both are smart enough to know they need to defeat an enemy as Reiner as soon as they can.
