There's a reason this "I don't get it" meme exists, and this chapter is a pretty good example. Tomo-chan can be really ambiguous and notoriously difficult to translate. Some chapters are actually really subtle, as well. I'm not gonna come in here and claim that I have any absolute answers, but I can clear up some major confusion for any of you nerds who don't get it at all.

Let's get the last chapter out of the way, first of all. Jun slapped the shit out of himself in attempt to remain composed while Tomo is all nude and squishy in his bath water. He is actively repressing romantic urges toward Tomo, and his fortitude is being severely tested. He's trying to shock himself out of lewd thoughts and back into his comfortable repression. In the first panel he's clearly shown lost in thought. The title is "My Best Friend's in My Bathroom". It's all pretty clear.

Now, we have two competing interpretations for this chapter, and obviously the jury is still out on the translation. The first, simpler interpretation is this one: >>138810855 where Tomo asks "why...?" and Jun is just apologizing for flinching and pushing her hand away.

The way I see it, there are two problems with this: first, there's no gag. There's not really a punchline in the final panel. It doesn't make sense in the format which has been established. Second, Tomo's reaction is over-the-top. She's not just shocked, she has an intensely suspicious, ominous look on her face. Look at all the shading over her eyes. Would she really react that way if she were just confused? Those eyebrows aren't just confused.

So now we've got the second interpretation, which may have been delivered in a sort of ham-fisted way here: >>138808042 In this interpretation Tomo has a suspicion about why Jun's cheeks are red. She thinks he's blushing, an indication of possible sexual arousal. She either thinks that he got all boned up because she was in the bath, or that he just pounded off to the thought of her in said bath. Which one you think it is should be governed by how extreme you think that facial expression of hers is. But keep in mind the whole nip arousal / blood pressure thing. That's the key here. Blushing, nose-bleeds, sexual arousal, all tied up together. Jun knows what she's thinking and is telling her that she's wrong. He doesn't want to explain it further if he doesn't have to because that will also be awkward, but he definitely doesn't want her thinking what she's thinking now.

I'm not saying one of these is right and one is wrong, I'm not fluent in Japanese, and we have to work with the translations we're given. I will say that the truth is probably somewhere in the middle of these two translations. I'm inclined to think that the suspicious interpretation is more accurate because it fits better. There's a more distinct punchline, and her expression makes way more sense.

Eventually Jun is gonna totally piss her off to the point where their friendship is in jeopardy. He's gonna offend her to the point where she challenges him to a fight. They're gonna fight for a while, Jun will come to the ironic realization that trying to hold onto their friendship by refusing any romantic entanglement has only pushed the two of them apart, that he's been a fool, and that he only has one road left open to him. You're right that this is kind of her MO. But I'd argue that that has pretty much only been when Misuzu has suggested she take sexual action. In this case she just has a vague suspicion that something might have been going on. Maybe she even suspects that he was peaking on her, that seems like a cliche enough trope that it could just be vaguely implied like this.
