The voice actor for Yang Wenli (of LOGH) died before the OVAs were done. They just used a different voice actor.

Happened to the MC of Honey & Clover for the last episode.

If it's a short series, chances are all the lines have been recorded already.

If it's a long-running series, they'll usually avoid using the character until they've found an appropriate substitute.

The VA for Irou died in production of book 3 of the last airbender, and they just wrote out Irou's spoken lines until they found someone else.

Otaku won't make a fuzz about a VA being changed if the VA in question is either: dead, seriously ill or retired. If that happens, they'll roll with a new VA being used.
Problem happens when the original VA of a character is perfectly capable for doing the job and willing to do it, but isn't cast.

I remember the guy who voiced Emperor in Aldnoah Zero died while it was still airing. But then his character died in the show as well, so they didn't need any more lines.
