Motoi's the most famous example because he drew Pretty Neighbor, which Azuma read. He was so impressed by his ability to copy his artstyle that I believe he prompted Dengeki to hook him up with a deal.

Koume Keito still does doujinshi.
Hiroe Rei does too.
Takeda Hiromitsu made one of my favorite saber doujins.
Yoshikadu prototyped Tsugumomo with an H-manga.
I've seen some crappy doujinshi from Akamatsu Ken.

Both Kiyotaka Haimura and Motoi Fuyukawa started doing porn before they started to illustrate Index/Railgun. In Fukuyama's case, he got popular in /a/ thanks to the Pretty Neighbor series, everybody was shocked when we learned that he was starting a manga in the same magazine that published Yotsuba.
