Arrow reflects are nearly 100% prediction, at most accurate ranges even superhuman reaction speeds aren't enough. Most pyros don't support their bow snipers so I think flaming huntsman reflect is one of the hardest icons to get unintentionally.

Arena was the least played gamemode. Even during its prime, Arena servers were either empty or had just 6 players.
A full server was incredibly rare.

No one really liked Arena. The problem is if you rush out to play, and die, you have to wait like 10 minutes to play again. So everyone would pussyfoot around to avoid death.
The meta was being engineer or medic and just not take any risks.

Not really. Most "good" Demoknight players I saw in games had a pub Medic up their arse. Sadly they can't do anything when their Medic die nor keep him alive beforehand. I say, playing a pocket Demoman effectively isn't as easy as you think. You may have more mobility than a Soldier on the ground, but lack of burst at range always will force you to leap with sticky jumps at long distances and move with a sweeper a.k.a second opinion pocket around you.
