Might and Magic series Wizardry series Dungeon Keeper 1 and 2 Mount and Blade Warband Banished Stronghold Crusader If the Monkey Island games count as medieval (pirates) those as well Bard's Tale Vampire the Masquerade Bloodlines is modern fantasy
The Witcher is easily the best when it comes to medieval settings and atmosphere. Dark Souls is actually very good with it's world building. Morrowind is fantastic, though not exactly "medieval". Mount and Blade is absolutely great, especially with mods. Gothic 1/2 are solid too. Fairly boring world, but well realized. And I'm hoping that the Kingdom Come: Deliverance will stay true to it's name and delivers what they promised.
