I think he's talking about that credits scene where some guy runs into the diner asking if anyone's seen Bing or something, then runs out. There's a hard cut but a few people are still in the same places. At the very end of the credits, that same guy gets up from a booth to the left side of the diner and casually walks to the front with a woman to pay. Thing is, that woman and a child are already sitting at the booth when that dude runs in, so unless his doppelganger was sitting at the booth and we couldn't see him, it's just a continuity error where they reused the same actor.

Either way, they left their kid at the booth.

The book is a revised edition of Secret History, which is itself a dossier. This is the "final" dossier, it's basically the same book but with extras and corrections. Lynch clearly had something to do with it, or Frost just stopped smoking whatever he was smoking and decided to do it right.

It's much more detailed than any of the other TP metafiction and a lot of it turned out to be semi-canon (Nez Pearce indians, greyliens, catastrophic fires, portals, everything that happens to Gardland at the end) so it's possible that it all adds up in the end.

the inconsistencies are likely Frost rewriting the show's plotlines to have more depth and meaning. There's not that many of them but where it diverges it doesn't break the show or anything.
