according to the amazon page, the title of the book (yes, it's a book despite it being listed as a dvd, although it does seem to include a disc of some sort) the title is ネットの超怖い話 which translates to "Super Scary Stories from the Web" so besides that I know nothing else.

In his defense, it is almost impossible to look up the source of that image. It will always be associated with either Gurgles and Bugman pasta or Ahenobarbus Henocied.

the source of the image doesn't really matter at this point, it's such a widely-used image and has been for so many years, and has been used by soooo many people trying to be spooky.
