Ive actually thought about this a lot, how the classic monster that draws on primal fear has an elongated face, sharp teeth, either piercing or black eyes, and a pale face, and it's all fairly easily explained, long face, sharp teeth, piercing eyes, think wolves, big cats, bears, and for a pale face and black eyes think of a stereotypical sick person, sunken dull eyes, deathly pale, both of these things would have meant death for you and everyone around you in primal times, so an instinctual fear of them was developed. A lot of the 'primal fear' are some things that are hard-coded into our brains and somehow made their way into our genetics.
People with no flight or avoidance responses to sick people/wolves as in your example are less likely to survive and pass those responses along.
Of course, these responses can be triggered by other things as well, such as the pale-face-dark-eyes images.

Cats are fuckin' terrified of cucumbers, apparently. (or at least some are). Probably a response to what the brain is hard-coded to interpret as a snake, and thus danger.
