Body modification has always been with humanity, ancient cultures throughout the world had piercings and tattoos, there have always been gay and trans people, they just have had to hide it (there's recorded examples of homosexuality in other species as well, there's also species that can naturally change their sex)
I would argue the rules of society are less repressive than they were in the dark ages (or even 100 years ago!), also globalization and increasing technology are a major factor (quicker cultural changes, and more time to discover ourselves)
